	Nominations must be made by a BOMA member in good standing.
	The current BOMA President is ineligible to receive an award.
	A panel of judges consisting of the BOMA Awards Committee Members will rate each entry, except those entries from their own organization.
	To nominate a BOMA member or yourself for one of these awards, fill out and follow the instructions on this form.
	Be specific when describing the qualities and assets of the nominee.

	Submit a copy of the nominee's curriculum vitae.
	Provide a reference from a coworker or supervisor to enable judges to obtain a perspective of the candidate in his or her work environment.
	Provide a tenant listing of key contacts and addresses. Indicate two tenants that should not be contacted. Two tenants will be sent a questionnaire.
	Submit a summary of nominee's leadership and innovative accomplishments in less than 600 words, if not submitted above.
